Agreement Between Patient and CureOn Medical Tourism Agency
1. Purpose of Agreement
The Patient seeks assistance in arranging travel, accommodation, and coordination for receiving medical or surgical treatment abroad. The Agency agrees to provide facilitation and coordination services but not medical advice or medical treatment.
2. Scope of Services Provided by the Agency
The Agency shall provide the following non-medical services (as applicable):
- Present treatment options and hospital/doctor profiles based on publicly available information.
- Facilitate appointment scheduling with healthcare providers.
- Arrange travel logistics (flights, airport transfers, hotels).
- Assist with visa documentation, if required.
- Provide estimated treatment costs based on information from the healthcare provider.
- Provide a patient coordinator or translator at the destination (if included in the package).
- Ensure communication between Patient and healthcare provider prior to travel.
The Agency does not provide medical opinions or guarantee treatment outcomes.
3. Responsibilities of the Patient
The Patient agrees to:
- Provide true, complete, and accurate medical history and documents.
- Follow instructions provided by healthcare professionals.
- Pay all required fees in accordance with Section 6.
- Obtain valid travel documents (passport, visa, insurance).
- Acknowledge that medical treatment decisions are made solely by the treating doctor/hospital.
- Inform the Agency of any changes in medical condition prior to travel.
The Patient’s failure to provide accurate information may affect eligibility for treatment.
4. Important Disclaimers (Non-Medical Nature of Agency Services)
4.1 No Medical Liability
- The Agency is not a medical provider.
- The Agency does not give medical advice, diagnosis, or treatment.
- The Agency does not guarantee the quality of medical care or outcomes.
- All medical decisions are made solely by the treating doctor/hospital.
4.2 No Guarantee of Costs
Quoted prices are estimates and may change based on treatment plan revisions, complications, extended stay, additional diagnostics, instruments, implants, or consumables. The Patient is responsible for all additional charges imposed by the healthcare provider.
4.3 Travel Risks
The Agency is not responsible for flight delays, visa issues, missed connections, loss of luggage, natural disasters, political unrest, or hotel, airline, or hospital service issues.
5. Medical Provider Selection
The Agency may recommend healthcare providers based on specialty, availability, and patient preferences. However, selection is ultimately the Patient’s decision.
The Agency does not guarantee credentials of the doctor or hospital, control medical procedures, or influence medical outcomes.
6. Fees and Payment Terms
6.1 Agency Service Fees
- Service fee of 10% of package.
- Additional charges for optional services (VIP transport, extended stay coordination, translation upgrades, etc.).
6.2 Payment Schedule
- Deposit upon signing the Agreement: 5000/- (non-refundable).
- Balance payable 15 days before travel / upon invoice.
- Payments to the healthcare provider are separate and governed by the provider’s policies.
6.3 Refund Policy
- Service fees may be non-refundable unless otherwise specified.
- Travel refunds depend on airline/hotel rules.
- If the doctor or hospital cancels, the Patient may reschedule or request a refund according to the provider’s policy.
7. Confidentiality and Data Protection
7.1 Patient Medical Information
- The Agency shall use medical records only for treatment coordination.
- Records are shared with selected medical providers only with the Patient’s consent.
- The Agency complies with applicable data protection laws (e.g., GDPR).
7.2 Confidential Business Information
Both Parties agree not to disclose confidential information obtained during this Agreement unless required by law.
